ข้าพเจ้า ได้รับคำชี้แจงจากอ๊อคซิแคร์ สหคลินิก หากมีผลตรวจพบเชื้อโควิด-19 ผู้รับการตรวจจะปฏิบัติตามแนวทางการดูแลรักษา ป้องกัน ควบคุม และการส่งต่อตามกฎหมาย โดยอ็อคซิแคร์ฯ จะต้องเป็นผู้ประสานงานจัดหาเตียงและโรงพยาบาลตามสิทธิ์เพื่อเข้ารับการรักษาให้ต่อไป และผู้รับการตรวจยินยอมเข้ารับการตรวจหาเชื้อโควิด-19 ในครั้งนี้ โดยความสมัครใจ และเข้าใจคำชี้แจงแล้วทุกประการ (I, hereby, receive an explanation from Occicare Medical Center. If I am tested positive for COVID-19, I must follow the guidelines for treatment, prevention and control according to the law.
